Regular - Primary Member

Youth Mbsh - Children 18 years and under can hold a Special Youth Membership without the head of the family holding a current Regular Membership. BIRTH YEAR MANDATORY

Family - Primary Member + 1 or more Affiliates/Dependents 25 years of age or younger who shares the primary residence.

Any dependents listed under a Family membership, MUST be listed under the parent or guardian’s Regular membership (not a grandparent living at a different address

****Include NAMES, GENDER, and BIRTH YEARS of ALL members in the “Add a note to your order” SECTION AT CHECKOUT. Birth years are MANDATORY.
