Reg - Primary Member

Affiliate - Primary plus spouse or 1 additional dependent 25 years or younger who shares the primary residence.

Family - Primary Member + 2 or more Affiliates or Dependents 25 years of age of younger who shares the primary residence.

Special Youth - Child 18 years or younger whom the head of the family is not a current Regular Member

 ****Include NAMES, GENDER, and BIRTH YEARS  of ALL members in  the “Add a note to your order” SECTION AT CHECKOUT.  Birth years are MANDATORY.

Any dependents listed under a Family membership, MUST be listed under the parent or guardian’s Regular membership (not a grandparent living at a different address)
